My Plan is founded on the belief that residents receive better care—and are happier, healthier people—when they are able to form  relationships with their caregivers. Under My Plan, each resident is assigned a primary nurse who personally leads the resident's care. This "consistent care" approach encourages residents and caregivers to form relationships and feel personally connected, resulting in care that addresses both health needs AND emotional needs. The "consistent care" component of My Plan was nominated by the Maine Health Care Association as a leading approach to care in the culture change movement.
